Community Mental Health Team
The team offers counselling for people experiencing stress or emotional difficulties including counselling for depression, bereavement, anxiety management, relationship problems and social support. Practice Counsellor
Our Practice Counsellor is Rachel Hodgkison. Please contact a doctor if you feel that you would benefit from a referral to the counselling service.

Midwife
Ceri Kilgallon is the community midwife attached to the practice and she can be contacted via the hospital on 01904 726 720 .

Community Matron
Jan Conde is a Community Matron attached to the practice. Her role is to look at a holistic approach to patient care, pulling together all the services that need to be involved in patient care eg social services, benefits advice, medical appliances etc.
All referrals to Jan are generated by the GP's and members of the nursing team.
