Out of Area Patients

If you live, or are moving, outside our Practice catchment area you may still apply to register with the Practice. It is very important that the individual health needs of patients can be safely met by their GP practice and this is not always possible when that is a distance from the patient’s home address. We will assess each registration to determine whether it is clinically and practically appropriate for you to be registered with us, and will inform you of the decision made. 

Patients who live outside of our Practice boundary may not be entitled to receive all the services available to patients within our boundaries, such as home visits. If this is the case we will advise patients prior to completion of their registration of any services we cannot provide them, and how to access them if required.

If you would like to apply to be an Out of Area Patient you need to complete a New Patient Registration Form and an Out Of Area form by following the links to the right of the screen or call in to the Practice and complete paper forms.  If you are already a patient and are moving outside our catchment area, you will just need to complete an Out of Area Form.
