New Patient Information- Regular Patients
Anyone who lives within our practice boundary can register as a patient with Elvington Medical Practice. The Practice is based in Elvington and accepts patients from adjoining villages.
If you live within our practice catchment area you will need to complete a New Patient Registration Form. You can do this by completing the form online by following the link to the right hand side of this screen, or by downloading the form and bringing it into the Practice.
We can only dispense medication to patients who live more than a mile from a pharmacy. This is a regulatory requirement over which we have no control. If you live within a mile of a pharmacy then we will write your prescription but you must collect your medication from a pharmacy, although this does not need to be the one closest to you.
When registering for the first time you will be invited for a new patient check. If you are receiving regular medication you will be invited to an appointment with the GP.

Medical treatment is available from the date of registration. Please contact reception for further information or click on the link on the right of the page. If you live outside our Practice catchment area and would like to become a patient, please see our page on Out of Area patients.
Our contract now requires us to inform all patients of their 'accountable GP'. If you would like to know who your accountable GP is, please give us a call and we can tell you this over the telephone. When making an appointment you can however specify any of our GP's that you wish to see, and we will try and accommodate you if at all possible.
